Yakovtsov Y. Surgical treatment of cholelithiasis in elderly and senile patients

The dissertation is devoted to the analysis of immediate result of open and laparoscopic surgical treatment for cholelithiasis in elderly and senile patients with establishment of pathogenesis of postoperative complications. On the basis of general clinical, biochemical, immunological, morphological and instrumental research, the age related features of immunological, hormonal and oxidative homeostasis as well as parameters of cardiovascular and respiratory system, which promote development of general and local postoperative complications, were determined. The conclusion about active surgical tactics with laparoscopic and endoscopic operations is made. With the purpose of prophylaxis of postoperative complications in patients with cardiorespiratory pathology, lifting technique, technique of programmed pneumoperitoneum with the use of the original apparatus for laparoscopic cholecystectomy and vita-melatonin in perioperative period, were applied, which significantly decreased death rate and incidence ofcomplications after surgical treatment of the patients elderly and senile age with the cholelithiasis.
