It is represented for defence 60 scientific works with investigation results of the general and regional anesthesia methods and organo-replacement intensive therapy influence on system and cerebral hemodynamics, oxygen status, fluid contents and fluid distribution in organism, integrated clinical characteristics in the peripartal period with 740 parturients and puerperas having severe preeclampsia-eclampsia. The comparative estimation of systemic and regional labor analgesia protocols, the general and regional anesthesia for cesarean sections has been carried out. The injuring role of labor- and operative stress in formation of infringements cerebral hemodynamics and development intra- or postpartum eclampsia is established. It is proved, that use of regional analgesia/anesthesia for delivery with women having severe preeclampsia resulted in 75-81% reduction of relative risk of postpartum eclampsia. Justification for application of organ-replacement technologies (the isolated or slow continuous ultrafiltration) with the purpose of elimination of critical frustration of an oxygen staus, cerebral blood circulation and cerebral metabolism, the fluid contents and fluid distribution in an organism at eclamptic puerperas resulting in 70% reduction of relative risk of maternal death was obtained.
