The thesis is devoted to the concept elaboration of the differential diagnosis of the organic and functional upper urinary tract flow impairment and to the pathogenetic grounding of their management options. According to the results of 285 patients clinical assessment and management with the obstructed uropathies the criteria for differentiation of the organic and functional upper urinary tract flow impairment variants were estimated in terms of their risk (high, low) assessment. The management options (surgery or monitoring) criteria have been proposed according to the estimated risk (high, low) results. The new approach to the operated kidneys management has been proposed on the basis of efficacy of the ACE inhibitors utility.
