The thesis solves an important scientific problem of surgery, i.e. substantiation of the methods of surgical intervention and principles of correcting therapy as well as investigation of mechanisms of development of post-operative complications and causes of unfavorable results of surgical treatment in patients with gastroduodenal ulcer. The work is based on the results of treatment of 426 patients with gastroduodenal ulcer. The author concludes that the choice of optimum method of operative treatment for ulcer requires complex examination with the investigation of the state of the upper portion of the gastrointestinal tract with the analysis of the data of clinical, x-ray, morphological and endoscopic examinations as well as assessment of secretory and motor function of the stomach and duodenum. Individual approach to the choice of the main and additional interventions in patients with gastroduodenal ulcer aimed at elimination of the revealed functional and morphological changes in the upper portionsof the gastrointestinal tract was suggested and introduced into the practice. The suggested algorithm of duodenum investigation which allows differential diagnosis between functional and organic disorders as well as clinical x-ray assessment of the state of these disorders in case when accompanying disorders of duodenal passage are suspected.
