Essay

In dissertation was substantiated necessity of intraoperative diagnosing of type and site of fascial and ligamental damage among woman with genital prolapse. It was obtained, that vaginal walls and cervical prolapse is a result of three types of fascial – ligamental damage - separation, rupture or stretching or in any combination. For each fascial – ligamental segment was proposed reconstructive surgery with reattachment or attachment de Novo. It was determined the place and role of each segmental reconstructive procedure and traditional operative management for cystocele, rectocele and uterovaginal prolapse. The choice of combination of procedures depending on appropriate diagnosing all combined damaging of three fascial – ligamental segments separately as advocated in proposed Segmental – Topic conception of choice of genital prolapse operative management. Excluding of unneeded reconstructive procedures and uterus preservation seems as main advantages for proposed segmental site – specific reconstructive surgery.
