Object of the study: Hypertension disease, chronic hepatitis, cirrhosis of the liver and their combination. Purpose of the study: To optimize the diagnosis and treatment of hypertensive heart by means of determining and correction of trophic and metabolic disorders accompanied by myocardium remodeling which is associated with disorders of the functional state of the liver in patients with HD accompanied by liver diseas-es. Methods of the investigation: clinical, laboratory, instrumental, statistical. Theoretical and practical results: The necessity to determine the hormonal state at various stages of HD was substantiated which expands the ideas about pathogenetic role of adaptation hormones in forming remodeled LV myocardium. The necessity to deter-mine different types of hyperproteinemia, glucose level and glycolized hemoglobin in the blood serum was substantiated. The necessity to determine the state of oxidation-antioxidation processes in patients with HD which influence the progress of metabolic disorders in the liver and structural functional disorders in the myocardium was proven. It was substantiated that to assess the state of cellular and humoral immunity it is reasonable to determine such markers as T- and B-lymphocytes, their subpopulations (helpers and suppressors), immunoglobulins A, M, G and cytokines TNF-? and IL-1?. Novelty: It was shown that dysfunction of neurohumoral systems is more frequently determined in liver disorders in patients with HD and is accompanied by correlation influence of adaptation hormones on geometrical parameters of hypertensive heart followed by structural functional changes of the LV myocardium. The influence of dysfunction of the hormonal state and activity of the pathological process in the liver in patients with HD on the character and degree of structural functional changes in the remodeled LV myocardium was established. It was proven that progress of metabolic disorders was accompanied by disorders of choleretic liver function with accumulation of bile acids and their fractions in the blood serum, which negatively influenced the state of trophic processes in the myocardium. The influence of subpopulations of cellular immunity on separate echocar-diographic parameters of the myocardium (circular fibers contrac-tion rate, ejection fraction) was noted. For the first time, it was shown that additional administration of essentiale forte N in com-bination with lecithin or thiotriasolin in combination with lecithin in these patients increases therapeutic effect with potentiation of ant hypertension activity of ACE inhibitors, restoration of the pa-rameters of remodeled heart, endocrine homeostasis, metabolic and oxidant-antioxidant processes in the organism. Positive thera-peutic effect in patients with stage II HD accompanied by CLD of lisinopril with thiotriassolin, berlition, lecitin was established for the first time. Degree of introduction: The findings of the research are used in the work of City Clinical Hospitals No. 11, 27, 1, 18 (Kharkiv), Kharkiv Regional Clinical Hospital, Central Clinical Hospital Ukrzaliznytsia, Dergachi District Hospital (Kharkiv Region), Cherkassy Regional Clinical Hospital, Ternopiul Regional Clini-cal Hospital, Regional Cardiology Hospital (Odesa), Institute of Gastroenterology (AMS), Institute of Cardiology named after M.D. Strazhesko. The findings of the research were introduced to the teaching and therapeutic activity of department on internal medicine No. 1, therapy of Kharkiv State Medical University.
