Essay

Object: multinodular euthyroid goiter (MNEG). Purpose of research: to study the clinico-morphological, immunological, and genetic characteristics of multinodular euthyroid goiter (MNEG), the factors that influence the results of surgical treatment, and develop on this basis adequate schemes of surgical tactics and prevention of postoperative relapse and hypothyroidism. Methods of research and the equipment clinical, ultrasound, cytological, histological, immunological, genetic, prognostic, and statistical methods; ultrasound investigation was being performed on an apparatus "Toshiba" (Japan) 01 MC 240 A; fine-needle aspiration biopsy was performed using an ultrasound puncture transducer "Alom-SSD-630" with a 7.5 mHz linear transducer. Theoretical and practical results: based on a retrospective analysis of the results of surgical treatment of MNEG, and on a study of morphological structure features, changes in hormonal, immunological status, genetic parameters, a mathematical model was developed forprediction of postoperative complications. This model may be a basis in the choice of an adequate volume of surgery so as to improve the remote results of surgical treatment of MNEG. Novelty: : it has been established that the incidence of development of postoperative relapse and hypothyroidism in patients with MNEG is a function of a cluster of factors, namely, morphological form of the goiter, volume of surgery, immune disturbances, genetic predisposition, terms of follow-up, and number of nodules in thyroid tissue. It has been demonstrated, using a correlation analysis, that immune disturbances represent one of the causes of postoperative hypothyroidism and disease relapse. A genetic and statistical analysis in samples of Ukraine and Azerbaijan populations has revealed differences in allele frequencies and differences in values of penetrances of homo- and heterozygotes.
