Essay

Subject: non-small cell lung cancer (NSCLC) of stage IIIA. Purpose: improve of short-term and long-term results of treatment of patients with NSCLC of stage IIIA for via the elaboration and introduction of methods of individualization of complex therapy taking into account clinical, pathomorphological and molecular-biological prognostic factors (PF) and chemoresistance markers. Methods: clinical, morphological, laboratory, instrumental, statistical. Results: the new approach to individualization of therapy strategy and prognosis for NSCLC of stage IIIA patients has been elaborated. This method forecasts the complex evaluation of main clinical, morphological as well as molecular-biological PF and facilitates the improving of the operability rate and patients' long-term survival. Novelty: the necessity of the platinum-based induction chemotherapy (ICT) as the component of complex treatment for NSCLC of stage IIIA patients has been grounded; the method for assessment therapeutic pathomorphosis has beenworked out; the authentic reduction of tumor's microspread into the bronchial wall after effective ICT has been proved, which promoted the elaboration of the original method of bronchial stump forming; the method of induction cytokinochemotherapy for NSCLC stage IIIA patients theoretically grounded and worked out; defined that adjuvant chemotherapy doesn't improve the relapse-free and long-term survival of radically operated patient; proved that positive PF is radical removing of the tumor, negative PF is the local spreading, N2-status and adenogenic histotype of neoplasm; defined the oncomarkers, allowing to forecast the clinical course and tumor's chemoresistance; worked out the mathematical models to forecast the oncoprocess course in phase of multimodality treatment for IIIA st. NSCLC patients, which allow make up a authentic prognosis and introduce the concept of "high and low aggressiveness of neoplasm's"; formulated the approaches to complex treatment individualization. The results of investigation hasbeen applied in thoracic department of Zaporizhzhya regional oncologic center, Kherson regional tuberculosis center, Donetsk regional clinical hospital, Dnipropetrovsk municipal surgical thoracic oncologic center, in educational process.
