Zhernov O. System of surgical treatment of deep thermal injuries of upper extremeties and their consequences

Object - patients with deep thermal injuries of upper extremities. Proposal - to improve surgical treatment from the moment of trauma till consequences removal on the basis of systemic approach using new pathogenically based surgical methods during all stages of burn disease. Methods - clinical research, biochemical, immunological, computional and mathematical modeling. Results - classification of postburn contractures of joints of upper extremeties was worked out on the basis of morphological characteristics and anatomical localization of injury; main features of tissue expansion process, of tissue transposition were determined experimentally. Рroposed system of surgical treatment of deep thermal injuries of upper extremeties and their consequences enabled to improve treatment results in 1,4 times, to decrease amount of deformations in 1,7 times and to reduce disability rate in 2,3 times. . Novelty - dermotension technology using expanded vascular tissues, methods of plastics of shoulder joints totalcontractures and flexion contractures of fingers with vascular flaps. Osteoartrogenic contractures of shoulder joints were treated by stable functional anchylosing of scapula and humerus with vascular flap plastics of injured site. Mobilizing technique and functional devices of external fixation were used in treatment of elbow joint artrogenic contractures.
