Heryak S. Structural-functional violations in the providing systems of pregnancy and births at women with subclinical hypothyroidism: prophylactic, medical treatment and prognostication of complications

On the basis of dynamic complex investigation of 274 pregnants with subclinical hypothyroidism and 52 euthyroid pregnant women with determination of thyroid gland hormones, TTG and ioduria, ultrasonography of thyroid gland and state of placental complex, fetal biophysical profile, violations of central and peripheral haemodynamic, activity of peroxide lipid oxidation and state of antioxidant systems, changes of cellular and humoral immunity, reactivity of the blood system, morphological, hystological and electronic-microscopic research of placenta were performed. Increasing of clinical signs of hypothyroidism with development of hypothyroid increase during pregnancy, anemic-hypoxic syndromes, secondary immunodeficiency, hyperperoxidation, systolo-dyastolic myocardial dysfunctions were established, which are the pathogenetic links of development and progress of placental insufficiency and fetal distres at pregnant with subclinical hypothyroidism. Adequate differentiated programs and algorithms of pregnancy' management according from different clinico-pathogenetic types of subclinical hypothyroidism were confirmed. Pregnants with subclinical hypothyroidism at with secondary immunodeficiensy and/or anemic-hypoxic syndrome a treatment complex with permanent hormonal L-thyroxin and Immunofan immunomodulator therapy is recommended. Pregnant with subclinical hypothyroidism with predominance of hyperperoxidation syndrome and/or systolo-dyastolic heart dysfunction in complex hormonal treatment by citoprotector with antioxidant properties Corvitin is necessary to indicate. Higher clinical effect of the offered programs with standard therapy of medical treatment comparing are shown here.
