Essay

The dissertation covers diphtheria treatment method improvement in children based on studying of clinico-immune-pathogenetic relationship, their pathomorphologic bases and mechanisms of diphtheria toxin damaging action under conditions of experimental diphtheritic intoxication modeling in Guinea pigs. Multiple organ injury pathogenesis, reasonable possibilities of their preventive and permanent correction, advanced paraclinical methods of clinical disease monitoring, treatment efficiency evaluation and recovery completeness were studied for the first time at the organ, cellular and subcellular levels. Determinancy of diphtheria clinical progress is grounded by a functional status of local defense mechanisms. Informativity of cardiointervalographic and electroneuromyographic study related to determination of subclinical level of vegetative and peripheral nervous system affection is determined. Ultrastructural mechanisms of diphtheria toxin damaging action against structural elements of myocardium, kidneys, vegetative and peripheral nervous system are detected under conditions of experimental diphtheritic intoxication in Guinea pigs. Immune complex genesis of glomerular filter hemocapillaries lesion is confirmed under conditions of repeated injection of high doses of antidiphtheric serum. Advisability of enterosorbents administration in multimodality therapy of diphtheria is proved.
