Garagatiy I. Orthotopic neocystoplastics: morphofunctional provement of choice of the surgical intervention method.

The theoretical generalizations and a new decision of a scientific problem are given in the dissertation. This problem consists of working out the concept of the individualized choice of the rational way of orthotopic neocystoplastics after radical cystectomy at the patients who need cystectomy. The decision of this problem has been achieved by studying multyfactorial interrelations among the kind of plastic material used during the reconstructive operation and its' performance-technics (by author's methods, as well as by known ones), frequency and structure of postoperational complications, terms of hystomorphological adaptation of neobladder mucous environmemt, biocenosis features, reservation- and evacuator-abilities of neobladder, functional status of kidneys.Criteria of choice of the plastic material were determinated on the basis of analysis of inspection of 179 patients with bladder-cancer and whom radical cystectomy with orthotopic neocystoplastics was performed. These criteria take in accountthe morphofunctional conditions and the technic-reconstructive features of the operation and their risks. The proved concept of individualized choice of rational orthotopic neocystoplastics-method is based on the different recovered advantages and lacks of each of these methods. Application of this concept provides the improvement of quantitative and qualitative result-parameters of specialized medical help to the patients with bladder-cancer.
