The study is devoted to the problem of treatment of patients with purulent disease of pleura. Peculiarities of morphological changes of pleural adhesions depending on duration of the disease and possibilities of use of proteolysis directed on decrease of fibrotic adhesive processes in pleural cavity were studied in experiment as well as the use of subpleural antibacterial therapy in patients with acute empyema of pleura. Diagnostic methods in patients with acute empyema of pleura were improved by introduction of ultrasound investigation of thoracic cavity. The method of controlled coagulation of pleural adhesions was worked out in experiment and applied in patients that allowed to decrease the number of complications caused by electrocoagulation action on nearby tissues in patients with acute and chronic empyema of pleura.
