Essay

Object of research is the diabetic encephalopathy. The work purpose - to optimise methods of diagnostics and to raise efficiency of treatment of patients with a diabetic encephalopathy on the basis of complex studying of indicators cerebrovascular haemodynamics, a functional condition of a brain, metabolic infringements and morphological characteristics of fabrics of a brain. Research methods - in a complex studied a carbohydrate exchange, lipid status, a condition of the free-radical oxidation, curtailing system of blood, a cerebral blood-groove, a functional condition of a brain, morphological, morphomethrical, statistical, mathematical modelling at patients with a diabetic encephalopathy and diabetes mellitus 1 and 2 types with different duration and complexity of disease (537 patients). On the basis of the carried out complex research studied indicators the new approach to diagnostics of a diabetic encephalopathy (on a basis mathematical modelling) which has allowed to establish the most informative indicators which influence on development of a diabetic encephalopathy is offered and allows to diagnose a diabetic encephalopathy on preclinic stages. Ways of diagnostics of brain blood circulation and bioelectric activity of a brain as methods of a determination of the diagnosis «a diabetic encephalopathy» are developed. Revealed on the basis of studying morphological material are established morphometric parametres cito-angioarhitektonik of a diabetic encephalopathy. On the basis of results of the carried out research strategy of pathogenetic therapy of a diabetic encephalopathy which includes correction of a carbohydrate exchange, hypothensive, hypolipid, vasoactive, neuroprotetive therapy is defined. Necessity of observance of a principle of timely treatment of a diabetic encephalopathy, i.e. for the period when possibility of functional restoration cerebrovascular haemodynamics and bioelectric activity of a brain is completely realised, against correction of metabolic infringements at patients with diabetes mellitus.
