Essay

The work is dedicated to the issues of discovering new pathogenesis pathways and tumor progress of uterine leiomyoma as well as optimization tactics of organ-preserving management. As a result of the research two clinical and pathogenetic variants of uterine leiomyoma were identified - isolated uterine leiomyoma and uterine leiomyoma accompanied by a dysfunction of mammary gland and thyroid gland, which to a great level differ by their mechanisms of development, speed of growth and morphological features. It was found that the leading pathogenic mechanism of isolated uterine leiomyoma is inflammatory process mainly of bacterial etiology and local dyshormonal processes, while in case of uterine leiomyoma combined with breast and mammary gland dyshormonal diseases distress, systemic hormonal, autoimmune and metabolic imbalances play the key pathogenetic role. Methods of diagnostics of the described above clinical and pathogenetic variants of uterine leiomyoma were refined. Created staged system of differentiated two stage approache to uterine leiomyoma management: pathogenetic treatment with consequent organ-preserving technologies.
