Theoretical generalization and new decision of scientific problem is in-process resulted in relation to determination of basic cardiovascular, hemodynamic and neurohumoral mechanisms of remodeling of myocardium for patients with hypertensive disease and determination of the most ponderable criteria for setting of the effective combined antihypertensive therapy. Work is based on information of clinic-instrumental-biochemical inspection of 381 patients with morbus hypertonicus II and III stages with the different degree of arterial hypertension and presence of the associated states. The predictors of gravity of structural-functional changes of myocardium and extracranial vessels at hypertensive disease and predictor resistant motion of arterial hypertension are discovered, the differentiated approaches are developed to therapy of found out violations by different combinations of modern antihypertensive preparations and ways of optimization already existent methods of treatment of patients with hypertensive disease of high and very high cardial risk are offered.
