Essay

The dissertation is dealing with the development and implementation into the clinical practice of the new ways of prophylaxis and treatment of postresectional and postvagotomic complications, optimization of complex preoperative treatment, intraoperative correction, postoperative treatment, development of new and modification of existing methods of operative interventions in case of the ulcerative disease of stomach and duodenum. Basing on the carried out morphologic, morphometric examinations, study of the motor-evacuatory, secretory function, regional blood flow, mineral density of the bone tissue the advantages of organ-remaining and organ-attenuated operations were proved. The principle new methods, directed on the prophylaxis and treatment of either early or late postoperative complications were developed and implemented into the clinical practice: method of continuous durable intragastric oxygen-therapy; method of peritoneum infecting prophylaxis after the surgical invasions on the gastro-intestinal tract; extracorporal ultraviolet radiation of the transfusion fluids; method of the endogenous intoxication correction while treating of perforative ulcers with the development of peritonitis and intestial impenetrability; method of choledocho-duodenal anastomosis forming, which prevents the reflux of duodenal contents not only into the bile ducts, but into stomach also; method of intestinoplication modeling in case of adhesive intestinal impenetrability development in the postoperative period; to treat the suppuration of the postoperative wounds was designed the bandage based on the zinc-gelatin with addition of extract of native barrios of conifers and hydrocortisone with the use of replaceable gauze linings-cartridges.
