Essay

On the basis of examination data obtained from 387 adolescents with ND there was proved a clinical polymorphism of the disease in the form of neurasthenia, depressive neurotic disturbances (DND) and phobic anxiety (PhA). By a cluster analysis data there was substantiated the expediency of singling out some leading complexes of syndromes in ND structure in adolescents in dependence on nosologic form of the disease and the age of patients. There were estimated catecholaminergic and serotoninergic systems, melatonin, ?-endorphines, leu- and met-enkephalines levels and there was defined their role in the mechanisms of developing some ND nosologic forms (neurasthenia, DND and PhA) in adolescents. There were obtained new data on the clinical structure and variants of neurasthenia, DND and PhA course in adolescents, depending on age and gender. Objective quantitative estimation of dynamics of the brain hemispheres' functional state was performed first on the basis of EEG spectral coherent analysis under conditions of simulated acute emotional "expectation stress". An algorithm for predicting neurasthenia, DND and PhA course in adolescents was worked out. There were also determined the most informative clinico-anamnestic, psychological, somatoneurological, neuropsychological and neurohormonal criteria as regards prognosis of ND course, depending on nosologic form of the disease. Prognostic tables which make it possible to work out individual prognosis were compiled, and the necessity of designing a system of innovative support of ND diagnosis and pathogenetic therapy in adolescents was substantiated.
