Essay

On the basis of analysis of results of 312 kidney transplantations a complex program of forecasting and diagnostics of kidney allograft dysfunction by morphological and noninvasive approaches is offered. Morphological changes in a donor kidney which program kidney allograft initial function, its function during the first year and life expectancy are determined. Histological and immunohistochemical criteria of diagnostics of kidney allograft dysfunction variants are improved. Тhe frequency of kidney allograft dysfunction variants is established and the structure is studied. It is proved that among the reasons of late kidney allograft dysfunction antigen-dependent factors prevail, namely humoral immunity. The survival of a transplant is defined by its function during the first year after operation which depends on the age of the donor, initial condition of a donor kidney, degree of its ischemic damage, presence of the delayed function and acute rejection in the postoperative period.
