Essay

The object of study: diabetes mellitus (DM) type 1 in children and adolescents. Objective: to determine the clinical features of the type 1 diabetes flow, the deepening of understanding of its pathogenesis in childhood and adolescence and the development of new technologies to optimize treatment of the disease in children and adolescents. Research Methods and equipment: physical, instrumental, biochemical, immunoassay, radioimmunoassay, immunological, statistical; "Cormay Multi", "Human", "TOSHIBA-400. Theoretical and practical results: The character of insular-сontrinsulyar relationships, their relationship to puberty are studied, features of immunological disorders in different parts of the system of protection of children and adolescents according to sex, degree of maturity of patients and duration of diabetes are shown. The data on the flow of type 1 diabetes in different age periods is received reducement of insulin sensitivity and increase in frequency and severity of diabetic complications during puberty is proven. The possibility of improving the compensation of carbohydrate metabolism in children and adolescents with diabetes, using insulin analogues, metformin, and cures for tireopaty is proven. Novelty: the first time to show that despite the substitution of insulin in one third of children and adolescents with DM type 1 there is a hyperinsulinemia, especially during puberty. Hyperproduction of leptin and IGF-1 is detected, the existence of sexual dimorphism overproduction of cortisol with a predominance of girls is proved. Persistence of antibodies to glutamatdekarboksilaze patients of childhood and adolescence and the importance of these changes to support persistent decompensation of the disease are determined. First time to obtain data on the thyreopathy frequency in children and adolescents with type 1 diabetes. Also shown that children and adolescents had violations of thyroid function in different directions, often represented by an increase in the most associated with decompensation of the disease. Degree of implementation: The research results are used in practice and research profile hospitals in Khakov's region and other regions of Ukraine. Scope: endocrinology, pediatrics.
