Essay

The thesis is devoted to solving the scientific problem of reducing maternal and perinatal morbidity and mortality due to obstetric complications, accompanied by endogenous intoxication syndrome SEI, through integratijn extracorporeal methods of hemocorrection into the system of organizational and therapeutic measures. It was supplemented and expanded data about the the pathogenesis of homeostasis disorders in PE and PID in obstetrics, confirmed the leading role of the translocation of gram-negative bacteria ET into the bloodstream, an imbalance in the system of endogenous and exogenous immunomodulators, placental factors. It was proved high informativeness of method of crystallography as a criteria for determination the indications for plasmapheresis and control the effectiveness of the treatment. It was shown that inclusion in the complex of intensive care of obstetric complications, which are accompanied by SEI, treatment PA is pathogenetically proved and leads to faster decrease of EI markers levels, normalization of homeostasis, prevention of PFS, pronounced improvement of clinical course of disease, prolongation of pregnancy, and as a result to reduction of perinatal morbidity in 1,5-2 times, perinatal mortality – in 2.8 times, complications in the postpartum period – in 4 times, duration of treatment in the department of intensive care – in 2,2-2,7 times.
