Essay

Object of study: the epidemic process of pertussis in the cell, organism, ecosystem, social and ecosystem levels, surveillance. Purpose: to establish the laws of the epidemic pertussis process in the present conditions and improve the surveillance system on the basis of analysis of the relationship functioning at different levels of the epidemic process of pertussis. Methods: epidemiological, serological, bacteriological, molecular genetic, clinical, statistical. Novelty: The efficiency of the current system of surveillance for pertussis and manifestation of the epidemic pertussis process in Ukraine under current conditions from the position of socially ecological conception were investigated. High efficacy of long-term planned vaccinal prevention of pertussis was established. It promoted stabilization of morbidity on a low level, prolongation of the cyclic periods, decreasing the differences between the level of morbidity of urban and rural population, changes of levels of age-morbidity. Features of serotype-specific view of the causative agent, clinical course of the disease, immunological structure of the population and their influence on the epidemic pertussis process were defined. Priority ways of improving the system of epidemiological supervision were worked out; reasonability of the use of modern laboratory methods of diagnostics was demonstrated and the criteria of their rational purpose have been established. The estimation of index of clinical severity of the disease, mathematical prognosis of prediction the tendency of morbidity on the basis of circulating serotypes of causative agents, the system of diagnostics and predictive criteria of epidemic well-being and precursors of epidemic problems were suggested. Results: the guidelines "Surveillance of pertussis" and a newsletter "A conceptual model of the epidemiological surveillance of pertussis infection" were developed and listed in the Register of Industry Innovations. Guidelines "Current methods of laboratory diagnostic tests for pertussis" was introduced to the work of six regional sanitary-epidemiological stations and in the learning process of four departments of medical universities of Ukraine.
