Essay

The work covers the topical problem of modern pediatrics science of improvement the efficiency of diagnostic tactics in various stages of neonates clinical monitoring hemodynamic who have perinatal pathology in early neonatal period on the basis of theoretical generalization and clinical research in hemodynamic disorders characteristics in adverse effects, creating the concept of their development and identification the patterns of progression, which has helped reduce infant mortality. The predictors of hemodynamic progression in infants were identified. The expediency of daily echo- and dopplerometrical monitoring of hemodynamic was proved. Characteristic modes of hemodynamic in the alteration of the fetal central circulation to adult type at birth of full term and preterm infants were determined. The changes of glomerular and tubular kidney function of neonates with hemodynamic disturbances were researched and the expediency of their determination to predict early neonatal period course. The levels of excretion of adrenal glands hormones of infants and their connection with hemodynamic disorders were determined, as well as special characteristics of the adenohypophysis morphofunctional state and adrenal glands of infants with perinatal pathology. The models to predict hemodynamic disorders of neonates for fatality involving clinical, laboratory and instrumental predictors of perinatal pathology progression were developed. Based on the analysis revealed modes of hemodynamic and its dynamic changes during the early neonatal period of neonates with perinatal pathology noninvasive diagnostic algorithms of hemodynamic disorders of full term and preterm infants are proposed, additional informative assessment criteria of hemodynamic depending on the type of respiratory child support, day of living and course of perinatal pathology are defined.
