The research has been devoted to clinical and immunological pathogenetic features study in post-stroke patients with hypertension stage III and obesity as well as prognostic criteria and principles of rational pharmacotherapy. The pathogenetic emergence concept of autoimmune cerebrovascular lesions associated with neurohumoral dysfunction and extracellular matrix degradation processes in patients with essential hypertension stage III in combined with obesity had been created. The role of neuropeptide Y, calcitonin-gene-related peptide in hypertension progression associated with obesity had been established. It was revealed the meaningful role of sympathetic hyperactivity in mechanisms of hypertension progression in obese patients. Close relationship between the obesity degree and risk of adverse clinical outcomes in post-stroke hypertensive patients had been established. It was determined that post-stroke cerebral lesions could had been an object of autoimmune activation strongly related to hypertension progression and increasing recurrent stroke risk of as well as cardiovascular events. The pathogenetic interaction pattern included autoimmune cerebrovascular lesions intensity correlation with neurohumoral factors and tissue biomarkers had been established. The prognostic values of the most meaningful clinical, immunological and neurohormonal factors of complications risk occurrence had been determined during long-term observation period. Optimal schemes of substantial treatment in post-stroke hypertensive obese patients had been suggested for improvement of secondary stroke prevention.
