Essay

The object of the study: insulinemia and androgen provision in patients with diabetes mellitus (DM). Objective: to study the gender characteristics of relationship between androgenic balance and insulinemia levels with markers of metabolic disorders in patients with type 2 diabetes, and also to investigate the methods of complex pharmacotherapy of this disease to improve the effectiveness of treatment. Methods and equipment: general clinical, hormonal, biochemical, immunoenzymatic, instrumental, radioimmunological and statistical; biochemical analyzers: BioSystems BTS-330, Shenzhe Mindray BioMedical Electroniks BA-88, Muller geratebau GMBH, SUPERGL. Theoretical and practical results: on the basis of a complex study of relationship between androgen balance indices and insulinemia in patients with type 2 diabetes their association with markers of hormonal-metabolic disturbances at different stages of the disease were demonstrated. These new data will differentially evaluate the revealed disorders and predict their development. These results demonstrated the expediency of a complex study of patients with type 2 diabetes at different stages of the disease - from compensatory hyperinsulinemia to insulin deficiency. The algorithm to determine the type of disorders in uric acid metabolism was proposed. It was recommended to concentrate attention on the use of pharmacotherapy of type 2 diabetes, the positive effects of which are not accompanied by increased level of insulin in the blood. Novelty: the relationship of androgen provision with insulinemia level, and hormonal-metabolic parameters in patients with type 2 diabetes were studied. Obtained new data have allowed significantly supplement the existing ideas about gender characteristics of hormonal-metabolic disturbances in patients with type 2 diabetes depending on androgen balance at different stages of disease development with different levels of insulinemia. It was indicated, for the first time, that disturbance of uric acid metabolism have sex differences and is manifested by hyperuricemia or hyperexcretory normouricemia in patients with type 2 diabetes. Degree of implementation: Practical recommendations were implemented into clinical practice of the departments of SI “V. P. Komisarenko Institute of Endocrinology and Metabolism, Natl Acad. Med. Sci. of Ukraine”. The main thesis of dissertation were proposed for use in the educational process at the Department of Endocrinology of P.L. Shupik NMAPGE. Area of application: endocrinology. Degree of introduction: the practice recommendations have been used in clinical practice of department of endocrinology of the Ivano-Frankovsk regional clinic hospital and the Central district hospital. The main results of study have been proposed for use in the educational process of the department of endocrinology of State institution “Ivano-Frankovsk National medical university ”. Field of application: endocrinology.
