Essay

Object of research: Acute and chronic recurring pyelonephritis. The aim - to raise the efficiency of treatment and to warn development of relapses in patients with pyelonephritis (PN) on the basis of definition the immunogenesis and uses the immunocorrection in complex treatment. Methods of research: immunologucal - ELISA, immunofluorescentical, common microlymphocyte toxic test (Terasaki); mycrobiologycal, biochemical, statistical. Equipment: microscope, thermostat, autoclave, dry-heat oven, biochemical and enzyme-linked immunosorbent analyzer, spectrophotometer, laboratory centrifuge, single-channel pipettes of variable volume. The patients with PN, against the decreased levels of CD4+ lymphocytes, CD4+/CD8+ ratio and phagocytosis indices, showed the links of immunogenesis - high levels of pro- (IL-1, -17, -18, -23, TNF, MCP-1) and anti-inflammatory cytokines (IL-4, TGF), SLPI, the number of ICAM-1+- lymphocytes and proapoptotic marker (CD95+). The high levels of MCP-1 in blood and SLPI in urine are considered as a predictors of acute PN /exacerbation of chronic pyelonephritis (CPN); the patients with recurring CPN are characterized by the high expression of CD54, CD95 adhesion molecules, and the TGF level (profibrotic for kidneys) in blood. There has been shown the high risk level for PN disease in the carriers of HLA- A10, -11, B14, -16, -17, and also the low expression of DR-antigens (<7%) on the antigen presenting cells in the third part of patients, that is the risk factor for CPN. The detected peculiarities in acute and chronic recurring PN allowed to formulate the conception of its immunogenesis. There has been determined in vitro the positive effect of immunotropic preparations on the functional activity of cells in patients in production of some cytokines. The received clinical immunologic effect of Galavit, Nucleinat, Manaxx and Proteflazid in treatment of the PN patients suggest the expedience of the planned schemes of their usage in the complex therapy. The degree of implementation: 4 patents, acts of introduction into teaching programmes of Ukraine universities and medical institutions. Scope (area) of application: medicine (surgery, urology, nephrology, gynecology, infectious diseases).
