Essay

The thesis is dedicated to the problem of improvement of diagnosis and treatment efficacy of patients with subarachnoid haemorrhage of aneurysmatic genesis. On the grounds of clinico-instrumental examinations of 350 patients in the acute period of intracranial arterial aneurysm rupture who underwent surgical treatment, an optimal diagnostic complex comprising, the peculiarities of the sickness clinical course have been investigated, the regularities of the development of cerebral vasospasm have been studied, developed a model of prediction of its severity and ischaemic complications with validation of the criteria of personalized approach to the treatment policy. The introduction of the set of diagnostic tests combined with ultrasonic monitoring and the proposed treatment algorithm made it possible to improve the outcomes of the treatment: increase the percentage of the patients who recovered, reduce the percentage of patients with disability, and the occurrence of delayed ischemic complications associated with vasospasm.
