Voloshyn A. Diagnostics and preventive maintenance of complications at laparoscopical gynecologic operations.

The dissertation is devoted a problem of decrease in frequency of complications at women operated by laparoscopical method concerning various gynecologic pathology on the basis of improvement and introduction of algorithm of diagnostic, treatment-and-prophylactic and rehabilitation actions. The total analysis of complications laparoscopical operations in gynecology is carried out that has allowed to establish risk factors, frequency and the reasons various intra - (traumatic) and postoperative (pyoinflammatory and trombohemorragical) complications. Frequency and structure of complications is shown at gynecologic laparoscopical operations of various severity level. The structure of gynecologic pathology at which it is necessary to prefer endoscopical to operative interventions is presented. Are established forecasting factors of development of infectious-inflammatory complications after gynecologic laparoscopical operations taking into account a condition of the microbiological status and character reparative processes in peritoneal exudate. Principal causes of bleedings which arise during time or after laparoscopical gynecologic operations at various genital pathologies are shown. It has allowed to define rational approaches to maintenance of safe and adequate hemostasis at laparoscopy. Treatment-and-prophylactic recommendations about optimisation of technics and technique of carrying out of the basic stages laparoscopical gynecologic operations of various degree of complexity are improved and introduced.
