Essay

In scientific work presents the results of a clinical study of 368 patients with complicated forms of peptic ulcer disease associated with reflux which made different types of surgical interventions by using of organ preserving and resection techniques. Learning basic etiopathogenetic factors of peptic ulcer and reflux disease (gastric secretory function, helicobacter contamination, undifferentiated connective tissue dysplasia), the main characteristics of gastro and duodenogastric reflux, the role of postprandial acid pocket allowed to develop surgical treatment of complicated forms of peptic ulcer disease, simultaneous correction of accompanying reflux and prevention of postoperative reflux. Developed of an antireflux techniques organ preserving surgery for the treatment of complicated forms of gastric and duodenal ulcer disease, antireflux techniques for prevention and correction of failure of gastroesophageal junction. Use of developed techniques allowed to reduce the incidence of postoperative reflux 3.7 times (p<0,01). Efficiency of the developed treatment strategy of complicated peptic ulcer disease combined with pathological reflux established based on the study of remote results of treatment of patients, that confirmed the reduction in the incidence of postoperative reflux from 62.9% to 16.8% (p<0,01) and a decrease in the number of patients with low quality of life after surgery from 26 1% to 6.4% (p<0,01).
