The paper deals with the problem of reducing the frequency of ovarian hyperstimulation syndrome and its complications by studying some new aspects of the syndrome pathogenesis, as well as the development and implementation of differentiated approach to the prognosis, prevention and treatment of OHSS. Gynecological complications of ovarian hyperstimulation syndrome have been subject to generalized analysis, which has allowed to define the risk factors, the frequency and causes of the syndrome. New aspects of OHSS pathogenesis have been studied by analyzing the interconnections between the case history, ultrasonic scans, Doppler flow studies, as well as lab and genetic tests. Also, a new mathematical model has been developed to detect groups of patients at high risk of ovarian hyperstimulation syndrome. The main risk factors for the development of various forms of ovarian hyperstimulation syndrome have been established, along with the criteria for taking prospective preventive steps to avoid the above syndrome, subject to subsequent assessment of the reproductive function. The gestation course, the nature of reproductive losses and the perinatal results have been evaluated depending on the time of OHSS onset (before or after embryo transfer). The sequence and methods have been proposed for taking the necessary diagnostic measures for various complications, which are beneficial for raising the efficiency of treatment and preventive methods of OHSS management. The paper defines the indications, contraindications and ways for optimizing the corrective methods for any detected complications of the ovarian hyperstimulation syndrome, which allows prevention of recurrent OHSS cases.
