Essay

The dissertation is dedicated to studying the clinical, neuroimaging, immunopathological manifestations of MS, forecasting its activity, and efficiency of pathogenetic therapy. Clinical and neurological manifestations of clinically isolated syndrome (CIS) were determined, as well as their correlation with the relapse frequency and degree of disability. The influence of relapses on progression of disability was studied. Dynamic changes in the number of T2 lesions, T2 lesions with a size more than 3 mm, T1- and Gd+ lesions and their relation with clinical manifestations of disease activity of different relapse rate and EDSS level were evaluated. The proportions of cerebral metabolites, subpopulations of lymphocytes and pro- and anti-inflammatory cytokines were determined, as well as their relation with clinical and MRI manifestations of MS. Clinical and instrumental indicators impacting disability progression and reaching of the certain EDSS level were determined. Тhe model of predictive assessment for achieving the moderate level of disability was developed. The efficiency of pathogenetic therapy of MS (first and second lines) and its impact on clinical and neuroimaging signs of activity was evaluated.
