Essay

The thesis shows new solution of scientific problem as the theoretical generalization of the findings of the investigation of mechanisms and factors that contribute to the development of obstetric and perinatal complications in HIV-infected women with associated herpes simplex virus infection and on this basis - pathogenetic rationale and developing the system of perinatal health of the fetus and newborn. The integrated analysis of the revealed findings demonstrates the perinatal pathology risk factors for the association with HIV and herpes simplex infection, among which the most important are indices of the infection activity, immune system status, hemostasis, cardiovascular regulation, microbiota, and fetoplacental complex. Evolved risk criteria were the basis for the development of predictive algorithm of the obstetric and perinatal complications. Established pathogenesis creates a theoretical basis for the development of pathogenesis-directed system of perinatal health of the fetus in HIV-infected women with associated herpes virus infection, with the inclusion of additional examination stages, the risk of complication predicting and differentiated set of therapeutic measures for eliminate detected disorders (antiviral, immunomodulatory, detoxication or endothelium-protective effect, biocenosis normalization, local immunity of mucous restoration).
