Pylypchuk V. Surgical treatment of patients with chronic pancreatitis complicated by biliary hypertension

The dissertation is dedicated to the improvement of the results of surgical treatment of patients with chronic pancreatitis with signs of biliary hypertension by improving the preoperative and intraoperative diagnostics, identification of nonmanifest hypertension, and intraoperative correction of surgical approach based on the proposed methodology. The method of intraoperative monitoring of biliary pressure is justified in patients with signs of manifest or nonmanifest biliary hypertension with the diagnostic aim; it allows for the study of dynamics of biliary pressure throughout the surgery on pancreas, adjustment of the surgical approach, and assessment of adequacy of the surgery on the head of the pancreas in terms of hypertension elimination. If biliary hypertension persists after the surgery on the head of the pancreas, the surgery should be supplemented by intervention on bile ducts. The indications to use each of the methods of surgical treatment of patients with biliary hypertension depending on the kind of other complications of pancreatitis and its spread on other organs are grounded in the research. The method of surgical treatment of patients with chronic pancreatitis complicated by biliary hypertension was developed and introduced.
