Thesis is concerned with the theoretical generalization and new solution of scientific problem regarding the improvement of the effectiveness of psoriasis vulgaris treatment based on the value of immunological and immunohistochemical reactions in the pathogenesis and morphogenesis of dermatosis.It was proposed an optimized scheme of therapy of patients with psoriasis vulgaris through the systemic immunobiological treatment course in combination with the course of the local narrow-band (311 nm) UVB therapy for locking an immune inflammation component important for the pathogenesis of dermatosis and sanitation of opportunistic pathogenic microflora foci under the stratum corneum in the areas of psoriatic lesions, that improve the effectiveness of treatment. It was developed an original diagnostic-therapeutic algorithm for management of patients with psoriasis through integrated general clinical and immunological examination of patients and specific morphological and histochemical study of biopsy samples from areas of skin psoriaform before and after a treatment. The proposed algorithm allows separately to determine the severity of the clinical course of psoriasis and the degree of epidermal hyperplasia and microbiocenosis violations in the areas of skin affected by psoriaform, as well as to assess the effectiveness of the treatment at the tissue and cellular levels, to predict the time to another clinical relapse of dermatosis and implement therapeutic interventions for increasing of remission duration.
