In dissertation сarried out a depth analysis and systematization of the local complications of acute pancreatitis for qualitative and quantitative characteristics. Identified the main characteristics of the modern pancreatic infection and their significance for estimation of severity of the clinical course of disease. Found the features of pH – metric and cytomorphologic researches data of peripancreatic liquid collections, organic affections of the digestive canal upper paths and pathomorphologic changes of the small intestine walls. Proposed diagnostic algorithm that promoted an increasing of precision of diagnostics and detection of local complications to 97%. Elaborated new kinds of medication therapy, in particular, leveling of the pain syndrome, elimination of exudative – edematous tissues affections, antisecretory therapy, correction of metabolic disorders, improved the methods of antibiotic therapy and early enteral feeding. Scientifically reasonable a surgical tactics using mini – invasive and traditional operative techniques as disconnectedly as together (in combination) that allowed to decrease an indexes of postoperative complications in 1,6 and postoperative mortality – in 1,5 times.
