Essay

The thesis is devoted the efficiency alloplasty of complex abdominal wall defects. Results of treatment of 379 patients with complex abdominal wall defects which carried alloplasty. Identified causes the formation of complex abdominal wall defects. Investigated the prevalence and severity of morphological changes of the tissues around the complex abdominal wall defects, their histological and immunohistochemical features in the area alloplasty. In the experiment, found out the effectiveness of the varieties of mesh implants alloplasty intraperitoneally with complex abdominal wall defects. By mathematical simulation modeling studied biomechanical processes and reliability criteria defined deformation of biological structures at alloplasty complex abdominal wall defects. Developed selection criteria exits complex abdominal wall defects. The possibility of using primary alloplasty complex abdominal wall defects in patients with increased risk of wound infection. Improved existing and new methods alloplasty complex abdominal wall defects. Efficacy of the type of location and method of fixing the mesh implant using laparoscopic alloplasty in complex abdominal wall defects. Worked out methods of prevention and treatment of postoperative complications, applied principles of multimodal applications "fast-track surgery" in the surgical treatment with alloplasty of complex abdominal wall defects, contributing to early rehabilitation of these patients. It is proved that a differentiated approach to complex surgical treatment of patients with complex abdominal wall defects, using methods developed alloplasty, prevention of postoperative complications and rehabilitation of operated patients improve the results of treatment: reduce incidence of wound complications to 5.2% versus 11.8%, total complications to 3.6% vs. 9.5%, 2.5% recurrence versus 6.7%.
