Babiy O. Diagnosis and surgical treatment of chronic pancreatitis at the stages of fibrotic pancreatic transformation (clinical and experimental research).

The results of clinical and experimental studies of surgical treatment of patients with chronical pancreatitis and fibrotic transformation of pancreas were presented. The main causes of unsuccessful surgical treatment of chronic pancreatitis and pseudocyst in pancreas were approved on the retrospective analysis.Clinical and morphological comparisons on the stage of fibrotic transformation of pancreas in chronic pancreatitis were provided.The specifics of fibrotic transformation of pancreas in experimental chronic pancreatitis model was researched. It was caused by disorders of NO-regulation system and pancreatic duct patency. Also, their relationship with biochemical parameters of inflammation and fibrosis was revealed.Validation analysis of visual and laboratory non-invasive markers of preoperative estimation of inflammation activity in pancreas with chronic pancreatitis on the stage of fibrotic transformation was realized in clinic. The algorithm of differential diagnostic was designed. New therapeutic tactics and new methods of minimally invasive surgical procedure were proposed. Also, their advantages in application for chronic pancreatitis treatment were evidenced.
