Essay

In the thesis for the first time, a systematized solution of the modern problem of medical psychology was proposed and implemented: a modern condition of pathomorphosis of cardiovascular risk (CVR) factors on the background of nonpsychotic disorders of the mental sphere (NDMS) was established and a system of their screening and psychocorrection was developed. Basing on the analizis of the frequency and nature of NDMS in young patients with different levels of CVR from the racours of clinical taxonomy, the direction and effectiveness of psychotherapeutic correction is proved. The further development of a new subject-oriented approach has also been made to integrate a patient-oriented strategy of psychoprevention, indication and psychocorrection with the implementation of medical and, at the same time, psychotherapeutic assistance at the primary and secondary levels of medical care. Practical approaches to the substantiated individualization of a patient with high CVR management is basing on the definition of the most common psychopathological symptoms of the NDMS have been processed. A multilevel screening system "pre-SCORE" has been worked out, which ensures the integration of psychotherapeutic techniques at various levels of medical care. The practical sanogenetic model of medical and psychological management of CVR in the system of patient-oriented preventive activity in the primary (general practition) and secondary (psychotherapist, medical psychologist) levels of medical care for the population is substantiated.
