Object of research: thrombotic lesions of the system of the lower vena cava. Methods of research: physico-mathematical calculations and experimentally investigated the model of arterio-venous fistula. Apply traditional and introduced new methods of instrumental diagnostics, including ultrasonic dynamic sonoеlastography, modified MSCT phlebography, radioisotope phlеboscintigraphy. The thesis presents theoretical generalization and new solution of a scientific problem - the inferior vena cava system thrombosis management improvement by the treatment and diagnostic algorithm optimization, surgical techniques improvement and methods of the venous thromboembolism complications prevention. The virtual model of arteriovenous fistula was created with aim of hemodynamics study. On the base of these results a fundamentally new methodology of arteriovenous fistula creation was developed. The AVF safe diameter was defined and justified. The method of dynamic elastography was implemented, which enabled to develop the embologenity criteria of floating thrombus. The treatment results of 1057 patients with the inferior vena cava system thrombosis were analyzed. Advantages of surgical treatment compared to conservative treatment were statistically justified, in relation to the pulmonary embolism prevention.
