Essay

Object - increasing of efficiency of treatment of malignant epithelial tumors of the eyelid skin Т2-3а stage based on a developed by radiocryosurgical method and comprehensive assessment of clinical, morphological, computer-tomography, immunohistochemical and immunological properties of the tumor, which allow to determine the degree of possible risk of recurrence. Purpose: improvement of efficiency of treatment of malignant epithelial tumors of the eyelid skin Т2-3а stage based on a developed by radiocryosurgical method and comprehensive assessment of clinical, morphological, computer-tomography, immunohistochemical and immunological properties of the tumor, which allow to determine the degree of possible risk of recurrence. Methods: visometry, palpation, biomicroscopy, ophthalmoscopy, clinical, morphological, computer-tomography, immunohistochemical, immunological, statistical; microscope "Janarued 2", spiral computer tomography "Asteion Super-4". There was established the possibility of individual forecast of recurrence risk by total scoring evaluation of the relative indicators of the expression of p53, bcl-2, CD95 and IPO-38 in the tumor tissues. The value of the amount above the threshold (>76,2 %) determines the probability of the risk of recurrence in 97,1 % of cases with specificity - 85,0 %. The method of treatment of malignant epithelial tumors of the eyelid skin Т2-3а stage consists of a gradual use of reduced doses of radiotherapy and cryotherapy was developed. It helps to reduce the total radiation dose of 1,5-2 times compared to pure radiotherapy and to increase the effectiveness of treatment of primary tumors to 94.6 % and jatrogenic - to 90,5 %. The results introduced into the clinical practice of The Filatov Institute of Eye Diseases and Tissue Therapy of the NAMS of Ukraine. Fields of application - medicine, ophthalmology.
