Badiul P. Reconstructive surgery of the wound defects with perforator flaps

The thesis is devoted to actual problems of improving the functional and aesthetic results of surgical treatment of patients with soft tissues defects of different etiology and localization by developing of complex program for the use of perforator flaps. The submitted study includes the results of the survey and treatment of 139 patients with soft tissue defects who underwent 195 reconstructions using perforator flaps. Based on the results of the study a comprehensive program of perforator flaps application was developed, including preoperative diagnostic preparation, the choice of flap type with the help of the developed algorithm, taking into account the anatomical and technical features of the harvesting of perforator flaps, the implementation of the dissection protocol, protocol of postoperative management of the patient, the algorithm of actions for microcirculatory disorders of flaps with the use, if necessary, of non-surgical methods for eliminating the consequences of the disturbance of the blood flow of the flaps. The effectiveness of the application of the developed program is proved.
