Dissertation is devoted to increasing the treatment efficiency and improving the quality of life of women with benign tumor-like formations of the ovaries on the basis of revealing of new aspects of pathogenesis and improvement of therapeutic, diagnostic and rehabilitation measures. It was established that the frequency of relapses of functional cysts after conservative treatment according to traditional schemes is 38,35 %, ovarian endometriomas - 35,03 %. After operative treatment, the relapse rate of functional cysts is - 13,53 % and of ovarian endometriomas - 5,88 %. The new links in the pathogenesis of benign tumor-like ovarian formations were found in patients with different types of ovarian cysts, depending on the reproductive function, there is a different type of immune response (Th1-, Th2 - type), the specificity of which allows to predict the effectiveness the chosen direction of therapy (surgical or conservative tactics). The typical Th1- / Th2 - type of immune response with the predominance of Th1 - type was observed in patients with functional cysts who did not give birth. The Th1 - type of immune response with an increase in the proportion of Th1 - type of cellular immune responses was diagnosed in women with functional cysts and infertility. The Th1- / Th2 - type of immune response with the predominance of Th1 - type was established in patients with functional cysts and realized reproductive potential. Using a personalized algorithm contributed to the realization of reproductive potential in 30 % of patients with with benign tumor-like formations of the ovaries.
