The dissertation is dedicated to the study of genetic-immunological mechanisms and regularities of the formation of acute edematous pancreatitis and exacerbation of chronic pancreatitis, taking into consideration polymorphism of PRSS1 (R122H), CFTR (delF508), IL-4 (C-590T), SPINK1 (N34S) and TNF-a (G-308A) genes, structural and functional changes in the pancreas, immune reactivity, cytokine profile, microbiota ecosystem of the colon, lipid and enzymatic misbalance. Genotype distribution was defined, their most frequent combinations in the structure of acute edematous pancreatitis and the exacerbation of chronic pancreatitis were analysed, as well as the interconnection with the character and the risk causes of the disease. The analysis of the chosen genes polymorphism influence on the pathogenetic chains of pancreatitis course was performed, the main laboratory-diagnostic peculiarities characteristic of some definite genotype, risk groups of patients according to the accompanying pathology, the appearance of the lipid dysbolism, the functional state of the liver and the pancreas, the large intestine microbiota ecosystem, immunological reactivity were distinguished.
