Hryvenko S. Reconstructive surgical methods for correction of the aesthetic (cosmetic) deformities of the anterior-lateral abdominal wall.

The dissertation is devoted to the development and implementation of the optimal techniques and tactics in the practice of reconstructive surgical interventions on the anterolateral abdominal wall. The experimental study substantiates the practicality of applying the proprietary technical solutions to improve the aesthetic and functional outcomes of reconstructive surgical interventions on the anterior abdominal wall. The algorithm of preoperative examination and preparation of the patient for the corrective surgery of the anterior abdominal wall was developed. The conceptual and methodical approaches to reconstructive surgeries for the cosmetic deformations of the anterior abdominal wall was defined. New methods of treatment for ventral, trocar and umbilical hernias are proposed. The promising outcomes of the combined graft fixation were proven. The degree of tissue injury was investigated according to the level of pro-inflammatory markers. An evidence of a higher degree tissue injury from the traditional interventions for the postoperative hernias compared to laparoscopic ones was demonstrated. Deficiencies of the immunogenesis were identified, and the intestinal therapy was proposed to prevent the endogenous intoxication and to avoid its progression. The specifics of early diagnostics, prevention and treatment of the pathological scars in the abdominal wall were presented.
