Essay

On the basis of the carried comprehensive study of the mechanisms forming hormonal-metabolic and hemodynamic disorders cluster in individuals with overweight body mass and obesity – representatives of Ukrainian population, a theoretical summarizing and new solution of topical scientific-practical issue, namely, deepening of the modern notion concerning the role of adipose tissue (adipokine imbalance) and modulating influence of extra-adipocytic hormonal disorders in the development of metabolic complications as the basis for comorbidities in this cohort of patients have been proposed. Object of the research: obesity and overweight body mass, their metabolic complications. The objective of the study – basing on the defined mechanisms of metabolic disorders formation in the patients with overweight body mass and obesity, to ground the concept of their prevention; to develop strategy and algorithm for early diagnosis of these conditions by determining population, clinical, hormonal, metabolic features in complex of their development in the urban population. Methods: general clinical, biochemical, enzyme immunoassay, instrumental, and statistical. Bioimpedance analysis of composition and liquid body sectors - monitor complex "КМ-АР-01 Diamant-P" (Diamant Ltd); rheographic method of registration of changes in arterial system - integral body rheography - computer rheoanalyser "Diamant-P" (Diamant Ltd). In the work, threshold values of adipose tissue accumulation in the body are identified for representatives of the Ukrainian population, which are associated with the degree of abdominal obesity and cause the development of inflammatory processes, insulin resistance, atherogenic dyslipidemia; diagnostically significant levels of circulating fractalkine, clasterin, vasipin and omentin for the development of insulin resistance of various degree were determined. Prognostic criteria for the development and progression of clustered metabolic and hemodynamic disorders in the individuals with overweight body mass of various degree are determined; the concept of their prevention strategy is grounded. The results of the research were introduced into the medical practice of endocrinologists, family medicine general practitioners in medical centres of Dnipro, Ivano-Frankivsk, Kropyvnytskyi, Kramatorsk, Mykolayiv, Severodonetsk, Kharkiv. Theoretical principles and practical recommendations are used in the lecture course, seminars and practical classes in Ivano-Frankivsk Medical University, Danilo Galitsky Lviv and Kharkiv National Medical Universities.
