Essay

Object – closed combined injuries of the chest and abdomen; goal – improvement of the effects of surgical treatment of patients with thoracoabdominal polytrauma and dominant breast damage through the development of new and improvement of existing technologies for surgical treatment; methods – clinical-statistical, experimental, clinical, clinical-laboratory, clinical and instrumental; results – for the first time, a clinical and epidemiological study of the characteristics of a modern joint thoracoabdominal trauma was carried out using the most informative statistical techniques, which made it possible to carry out a risk-oriented clinical and epidemio-logical analysis and to identify ways to improve the diagnostic and therapeutic tactics of severe closed combined injury of the chest and abdomen on randomized clinical and epidemiological groups; for the first time, randomized clinical and epidemiological characteristics of the traumatic process in victims with combined thoracoabdominal trauma were established; significantly supplemented scientific data on the clinical-nosological and clinical-anatomical characteristics of injuries in victims who survived and died in a combined thoracoabdominal injury with dominant breast damage; for the first time, objective criteria were established for the factors that created the risk of death in combined thoracoabdominal trauma; for the first time, it was scientifically justified to combine all components of the operative correction of injuries of the chest and abdomen into a single thoracoabdominal operative reception with the development of new and improvement of existing treatment methods based on a comprehensive clinical and epidemiological analysis of modern joint thoracoabdominal injury; for the first time, a kinematic analysis of the flotation process of the chest and an experimental comparison of the biomechanical characteristics of the treatment of damage to the sterno costal framework was carried out, which made it possible to justify the development of new devices and methods of treatment at a new scientific level; for the first time, new technologies for the operative correction of damage to the sterno costal framework were developed and substantiated on the basis of a carefully planned and conducted at a high scientific level experiment; branch – medicine.
