Object - an early neonatal period in healthy donated neonatal children and children with manifestations of perinatal pathology; the goal is to increase the effectiveness of providing medical care to newborns with impaired functional status of the urinary system in the pathology of the early neonatal period by improving the methods of prediction and diagnosis on the basis of studying specific biochemical and functional markers and the creation of a differentiated treatment algorithm taking into account the severity of renal dysfunction; methods - clinical, questionnaire, epidemiological, biochemical, immunochemical, photometric, instrumental, statistical-analytical; results - the peculiarities of the functional state of the urinary system in newborn infants under conditions of physiological adaptation and violated course of the early neonatal period on the background of abnormal postpartum oxidative stress, taking into account the severity of perinatal pathology; The state of glomerular filtration on the level of cystatin C in neonatal blood with pathology of the early neonatal period of various degrees of severity was studied; A high diagnostic value of uric lipolin, associated with gelatinase of neutrophils, was established for the detection of renal dysfunction with violations of postnatal adaptation of moderate severity; found that renal disorders in children with perinatal pathology of moderate severity are characterized by tubular dysfunction, severe degree - a combination of glomerular and tubular dysfunction; The complex algorithm for prediction, diagnosis and differential diagnostics of the mild, moderate and severe degrees of gravity of disorders of the functional state of the urinary system in newborn babies with pathological course of the early neonatal period is proposed with the help of discriminant analysis of the most important indexed perinatal risk factors and clinical and paraclinical markers of renal dysfunction; A system of differentiated therapeutic measures aimed at correcting disorders of the functional state of the urinary system and preventing the formation of acute kidney damage in newborn babies, taking into account the severity of perinatal pathology and the depth of renal dysfunction, was developed.
