The dissertation to theoretical synthesis and new practical solution of the actual problem of diagnostics and complex treatment of surgical nail pathology, including complicated, combined and recurrent defeat of nails and paronychial structures defeat by developing of clinical classification criteria, methods of surgical intervention, optimization of the choice of the sequence of application of some different methods, of effective schemes of antirelapse additions of clinical measures and prevention of early and late postoperative complications is devoted. The materials of the dissertation on the results of the retrospective and prospective analysis for the 10-year period (2006-2016) of medical cards of 919 patients are based: 503 men and 416 women aged 5-92 years old who at the outpatient surgical departments were treated. All patients into 2 equal groups by age and nosological forms were divided. The highest frequency among the technical causes of relapse was ascertained as a result of the refusal to carry out partial matrixectomy — 19.8% of the subsample and traumatic onichectomy — 17.25% of the subsample. We have proposed methods for the surgical treatment of onycheal incarnations. The use of partial matrixectomy is recommended in all cases (under conditions of mycological contamination - by diathermocoagulation) as the main anti-relapse measure, which is a component of three-component surgical interventions that reduce the number of postoperative compression relapses,? 2 = 31,23, p <0,01, to 1 -3% of the total sample of clinical observations.
