Essay

The dissertation presents the theoretical substantiation and practical solution of the actual scientific problem: increasing the efficiency of surgical treatment of patients with liver cirrhosis by optimizing tactics and the methodology of their surgical and complex conservative treatment. Formation of the multiple organ failure syndrome, portal hypertension, ascites and cholestatic syndromes in this contingent of patients should be borne in mind during the planning of individual tactics for their surgical treatment. It is proved that operations of selective and partial porto-cavity shunting with the purpose of correction of portal hypertension in patients with cirrhosis of the liver are an effective method of treatment of the underlying disease. It is established that complete devascularization of the stomach, together with the operation of Paciory, is the best methodological method for the surgical treatment of patients with liver cirrhosis at high risk of bleeding from varicose veins of the esophagus and stomach. Significantly better quality of life has been achieved with the use of minimally invasive technologies and inclusion of laparoscopic surgical interventions about liver cirrhosis and its complications. It has been proved that the quality of life in operated patients is better compared to that in patients with liver cirrhosis who used open surgical interventions.
