Essay

The dissertation is devoted to the complex research of prediсtive factors of combination of schizophrenia with somatic pathology; clinical and psychopathological and pathopsychological transformations of schizophrenia under the influence of comorbidity; social and personal functioning, quality of life and compliance of patients with schizophrenia and somatic disorders; awareness, attitude to illness, presentation of somatic and mental illness in the minds of patients, which became the basis for determination of the prognosis and development of a comprehensive program of therapy and rehabilitation of patients with schizophrenia and comorbid somatic pathology. The study was based on a survey of 186 people with schizophrenia (F20), of whom the main study groups were 136 people with chronic somatic diseases: 50 patients with cardiovascular disease (CVD), 42 patients with type 2 diabetes mellitus (DM 2) and 44 patients with obesity. The control group was represented by 50 patients with schizophrenia without chronic somatic diseases. To achieve the goal and tasks of the study, a set of methods was used in the research, among them: clinical psychopathological, psychometric, psychodiagnostic, mathematical and statistical. It was revealed that the combination of schizophrenia with somatic diseases lead to more malignant variants of the course of psychotic disorder, in particular, an increase in the number of hospitalizations, earlier manifestation, prolonged duration and a greater severity of psychopathological symptoms. It is established that schizophrenia with comorbid somatic disorders are associated with negative clinical and social prognosis and can cause intensification of psychopathological symptoms and reduce the level of social and psychological functioning. Taking into account the identified features of clinical and psychopathological manifestations, pathopsychological mechanisms of awareness, perception and attitude to mental and somatic illness; socio-personal functioning, compliance and quality of life of patients with schizophrenia and somatic disorders, it was developed and tested in practice a system of complex therapy and rehabilitation of these patients, which was implemented on the basis of a comprehensive approach using psychopharmacological, psychoeducational, psycho-therapeutic and psychosocial methods, which were selected with pathoplastic of psychopathological process when it was combined with somatic pathology. According to the results of approbation of the developed system, its high efficiency is proved, which is confirmed by positive changes of clinical and psychopathological manifestations and improvement of social and personal functioning.
